Club Representative form 2012
The club rep form is available to complete on line, (in the left column after logging on, or from the "Download" link at the top under registration forms. I have only received 35 forms by 22-Jan.
The AGM is on Monday 23rd January, all clubs to submit by this date. If your club AGM is after this date, therefore do not have confirmed captains, please state in the appropriate section "To be confirmed". Then submit the form again once you have this detail.
I will advise soon of the deadline for re-submission in order for the handbook to be completed and printed.
All forms downloaded to be completed as an Excel sheet and returned to This e-mail address is being protected from spambots. You need JavaScript enabled to view it. saved as your clubs name.
All forms submitted on line are removed within 24 hours and stored on league software only.
